Chilliwack – The Fraser Valley Health Care Foundation (FVHCF) is looking to renovate the current Primary Care Clinic at Chilliwack General Hospital. A partnership between the Chilliwack Division of Family Practice and Fraser Health will bring an improved Chilliwack Primary Care Clinic (CPCC) at CGH. Launched in 2011 and located in Chilliwack General Hospital, it provides access and comprehensive care five days a week from 9am-3pm.
The Chilliwack Primary Care Clinic provides care to some of the most complex and vulnerable patients in our community, many of those who were familiar faces in the Emergency Department and have mental health and/or substance use challenges. The Clinic is also closely linked with the Division’s local patient attachment mechanism and Recruitment and Retention Committee to facilitate attachment to both the Clinic and to community practices (over 6000 patients to date).
The Clinic supports team-based care with a compliment of Nurse Practitioners, General Practitioners inclusive of those specializing in mental health and substance use), a mental health clinician, a psychiatrist, and a clinical pharmacist. The interdisciplinary team accepts referrals from community for comprehensive medication reviews and take-home naloxone. It links with the hospital for early discharges to support follow up for unattached patients, as well as other specialized clinics such as the seniors’ clinic, the diabetic, respiratory and cardiac clinic.
Renovations would support the expansion of the current Clinic team to add family physicians, nurse practitioners, social work, and access to additional mental health supports and a community rehab team. This expanded team would support increased attachment, a greater focus on chronic disease management, and outreach to the Chilliwack Youth Health Centre (providing primary care and mental health access to youth and young adults up to the age of 26) and to street entrenched clinics.
The Fraser Valley Health Care Foundation is committed to raise the $1,000,000 to fund the renovation of the Primary Care Clinic in order to service detached patients. The Foundation will endeavor to raise the funds over a 2 year period ending March 31 2021.
